Manual Data Entry & Document Reconciliation (Handwritten Records) Project Overview I am looking for a highly detail-oriented freelancer to manually review approximately 3 months of handwritten records and transfer information into a master audit document. This is NOT an AI or OCR task. AI has already been used to create the initial master document, however many handwritten entries could not be read correctly. This project requires careful manual review of multiple source documents. Scope of Work You will be provided with: A Master Audit document (Word) Daily Food & Fluid Intake charts Daily Handover Sheets Other supporting records where required The project covers approximately 3 months of documentation (around 90 days). For each day you will: Compare all documents for that date. Enter any missing information into the Master Audit. Complete missing staff names by cross-referencing the Daily Handover sheets. Identify missing meals, fluids, urine output and documentation gaps. Highlight any periods where documentation appears incomplete. Ensure all entries are in chronological order. Maintain the existing formatting of the master document. Examples of Work Required The AI-generated document currently contains entries such as: "AM Staff - Not Legible" "Food not recorded" "Missing meal" You will need to locate the original handwritten record, identify the correct information and update the Master Audit. For example: Identify which staff member worked that shift. Read handwritten meal entries. Confirm fluid amounts. Record urine output. Fill in missing times. Highlight genuine documentation gaps. Nothing should be guessed. If handwriting cannot be confidently read after checking all related documents, simply mark it as Illegible. Skills Required Excellent attention to detail Strong English Experience with Word Comfortable reading handwritten records Experience with data entry Able to compare multiple documents and identify inconsistencies Experience with medical records, disability services, aged care, legal document review or auditing is highly desirable. Deliverable A completed Master Audit document with: Missing data added Staff names completed Documentation gaps highlighted Consistent formatting maintained No assumptions or guessed information Estimated Project Size Approximately 3 months of records 17 pages of data Around 90 daily charts, plus supporting documentation To Apply Please include: Your experience with handwritten document transcription.
