**Scope of Work – Employment Verification Site Visit** We require an independent employment verification for a candidate who has claimed employment with the following institution: **National Cheng Kung University Hospital** **No. 138, Shengli Road, North District, Tainan City 704302, Taiwan** Your task is to conduct an in-person visit or, where appropriate, contact the relevant Human Resources or authorized department to verify the candidate's employment. The objective is to confirm the individual's employment and obtain any information that the institution is authorized to disclose. **Required Verification Details:** * Confirm whether the individual was employed by the institution. * Verify the exact job title/position held. * Verify the employment start date and end date, or confirm if the individual is currently employed. * Obtain any additional non-confidential information the institution is willing to disclose (e.g., department, general duties, employment status, etc.). **Required Deliverables:** * Complete our employment verification form. * Obtain the institution's official stamp and authorized signature on the verification form, if permitted. * Record the date and time of the verification call or site visit. * Provide the name, designation, and official contact details (phone number and/or email address) of the representative who provided the verification, where permitted. * Prepare a concise written summary of the verification outcome. * If the verification is conducted in person, capture **6–7 clear geo-tagged photographs** of the facility, including the building exterior, hospital signage, main entrance, reception area (where photography is permitted), and any other relevant areas. * Submit all photographs, the completed verification form, and the verification summary upon completion.
