For my current immigration case, I must submit a formal, clinician-signed mental health evaluation. The assessment will need to be completed from scratch because I do not have any prior psychiatric records or past evaluations to share. Scope of work • Conduct a comprehensive psychiatric interview (video consultation is acceptable if you are licensed to practice and issue reports remotely). • Administer standard diagnostic tools you feel are clinically appropriate. • Prepare a clear, professionally formatted report that meets typical immigration-court or consular standards—covering history, clinical findings, DSM-5 diagnoses (if any) and treatment recommendations. • Include your full credentials, license number, date, and signature on the final PDF. Acceptance criteria • Report is delivered in PDF within the agreed timeframe. • All sections required by immigration authorities are addressed. • Language is concise, objective, and free of jargon, suitable for legal review. Please indicate your licensure jurisdiction, typical turnaround time, and any additional information you need from me so we can schedule the evaluation promptly.
