I need a digital disclaimer form that guests can read, sign and submit directly on an iPad. The form must collect three sets of information: their personal details, explicit agreement to the terms, and a health & safety acknowledgment. When a guest taps Submit, three things should happen automatically: a confirmation email goes to the guest (and a copy to me), a PDF of the signed form is generated, and all data is stored in a database for easy retrieval. I only have a rough visual idea, so I’m counting on you to translate that into a clean, touch-friendly layout that feels at home on an iPad screen and guides the user smoothly from start to finish. The signature field needs to accept finger or stylus input, and the whole experience has to work offline first, then sync as soon as the device reconnects. Deliverables • Responsive iPad-ready form with signature capture • Automated workflow: email confirmation, PDF generation, database storage • Source code, database schema and a brief hand-over guide If you have experience building kiosk-style web apps or native iPad forms with features like HTML5 canvas signatures, JavaScript/PHP back-ends, or Swift/SwiftUI front-ends, that will help us hit the ground running.
