I need a clean, fully-functional order form built in JotForm that I can link directly from my website. The form must collect standard customer information and allow shoppers to choose from our product options, then pass the data straight into PayPal so a quote or invoice is generated automatically. Key points • Fields: customer contact details plus selectable product options exactly as shown in the sample order form I’ve attached. • Email workflow: every successful submission should trigger an automatic confirmation email to the buyer. • Conditional logic: I’m not certain yet whether we’ll need any; please look at the example and advise the simplest approach. If logic will streamline the experience, include it—otherwise keep the form straightforward. Deliverables 1. A live JotForm that mirrors the attached example and is connected to my PayPal account for seamless quote / invoice creation. 2. Embed or share link code so I can place the form on my website. 3. Brief instructions outlining how I can edit products, prices, or email text in future. If anything in the reference form is unclear, let me know in your proposal.
