I need a clean, well-structured datalist that covers every registered hospital, nursing home and clinic operating in the north-Indian states of Punjab, Haryana and Uttar Pradesh. Scope of work • Source the most up-to-date information from government health portals, state medical councils, NABH listings, local directories or verified open datasets. • Capture, at minimum, the facility name, full postal address, city, contact phone, email (if available), ownership type (public / private / charitable) and any accreditation noted in the source. • Present the final data in both Excel and CSV so it is ready for import into our internal system; please keep one row per facility and use consistent column headers. Acceptance criteria 1. All three states (Punjab, Haryana, Uttar Pradesh) are represented. 2. Each facility is categorised correctly as Hospital, Nursing home or Clinic. 3. No obvious duplicates; basic deduplication checks should be performed. 4. File passes a quick spot-check of 30 random entries against the original source links you provide in a separate “Sources” sheet. If you already maintain a similar database, let me know—speed is welcome as long as accuracy is maintained.
